Did not ask for email or phone number. If it is a scam its a lame one. They can send mail to my house. I recycle so no big deal.

It is a scam. I did a Whois on the domain name. This same person also has one up for Godiva Chocolate. This form is freefloating in cyberspace with no homepage, just a form for your info, and hoping u click a googlead, so their pocketbook gets a little bigger to do this again and again and.....
