I got this today and there was nothing about the downloads. Is this the one for this post or the clinical that was for the post where you text in for it. It was full sized too.:)

finally got this Friday... thought I wouldn't because everyone else got this so much earlier than i did. :teeth:
 
Just a note to those that couldn't get to the actual sample form... I did the copy and paste thing and for a week I had that problem but I just kept trying each day and after 5-7 days of my attempts being fruitless, it just suddenly worked!
(not even sure if it's still valid but if it is, i recommend trying until it DOES work!)
